Based on the evidence obtained during a DFPS investigation, a caregiver was not demonstrating self-control when they were yelling shut up in the presence of children in care.

Based on evidence found by DFPS, a child in care was forced to sleep when caregivers, at different times, put the bottom part of their legs over a child's body, to try and keep the child still to fall asleep.

Based on the evidence obtained caregivers did not use good judgement when patting a child's back hard, causing bruising and petechiae. The child expressed discomfort by crying and asking them to stop and they did not.

Based on the evidence obtained during an investigation, operation staff allowed a child under the age of two to participate in screen time on 2 separate occasions.

Based on the evidence obtained during an investigation, operation staff failed to ensure that they communicated with one another prior to leaving a classroom all incidents and injuries involving a child in care resulting in parents not being notified timely of the incidents. .

It was found during the investigation that caregivers were allowing infants to sleep in a swing.

In the infant classroom, there was a 3 month child asleep that was placed on there tummy who is not independently able to roll on there back. CI: This was corrected at the inspection as the child was place on there back.

There was no food allergy plan avaiable for review for a child with a known food allergy.

During the investigation, it was determined that the operation did not properly notify parents within 48 hours of receiving notification of a communicable disease within the operation.

During the investigation, it was determined that the operation did not report a notifiable communicable disease to Licensing within 48 hours of being notified.
